Appointment Request

Thank you for selecting my practice!      To schedule an appointment, please call our office at 770-396-2206 or you can submit a request for an appointment as follows:

1)Proceed to the Home page of this website
2) To your left you will see a menu-scroll down until you see “Patient Forms & Policies”
3) Click on “Patient Forms & Policies”
4) Complete the “Reason for Visit” form and press “Submit.”
5) Next, choose the Informed Consent form for either “Treatment/Consultation”  or  “Evaluation” and complete then press “Submit.”

We will receive your forms after your press Submit and will confirm an appointment date for you. If you have any difficulties, please call our office at: 770-396-2206 or send us an email message by completing the below note box. 

If you receive a voice mail when you call our office, we are either extremely busy and handling another call, or you have called after hours. Please leave a message with your contact information so that we may return your call. You may also want to consider sending an e-mail to: [email protected]
